All patients of EYE REPUBLIC
Ophthalmology Clinic will be given the
LOYALTY card.

The patient will be given
a LOYALTY card
upon registration at the reception area.
Should there be additional areas
where a patient can obtain a
LOYALTY card, we will announce accordingly.

The card will be issued to the
patient at the time of registration starting 01 March 2006.

No fee or payment is required to
obtain a LOYALTY card. The only requirement is to be a registered patient
of the EYE REPUBLIC by availing any service provided by the EYE
REPUBLIC, i.e., outpatient services, emergency service, and
admission.

The following benefits will be
given to the LOYALTY
Cardholder:
Prompt Registration
20% CASH or 15% CARD DISCOUNT on the following services:
Outpatient diagnostic
services (keratometry, A-scan ultrasound, biometry,
pachymetry, Humphrey visual field, digital photo and video
documentation) – only where available
Outpatient consultation
and procedures professional fees
10% CASH or 5% CARD DISCOUNT
for outpatient optical (including frames, prescription lenses,
sun glasses and contact lenses)
10% CASH or 5% CARD DISCOUNT
for surgical professional fees – only when fees are paid in
full at least 1 day before the surgery

LOYALTY Card must be
presented by the Cardholder to the reception staff at the point of
registration for the to be extended.

The
LOYALTY card
benefit/discount is not applicable for Emergency Room services and
Inpatient services. The discount only applies to the services
mentioned in number 5 when done on an outpatient basis and
outpatient pharmacy purchases.

No. Only one discount scheme is
extended to any LOYALTY
Cardholder. We will use and extend the Senior Citizen's discount for
the eligible elderly or senior citizen patients.

No. Only one discount scheme is
extended to any LOYALTY
Cardholder. The patient may use either discount scheme, but not
both.

No. The 20% discount is not
applicable for promotional discounts, special offers, and packages
such as refractive holiday packages.

by 5% with payments by all credit cards that the clinic accepts.
