To make your clinic visit more expedient and worthwhile,
we have taken the necessary steps to provide you with much needed
information. In this section, you will learn about your billing statement
and payment. Tips on how to set, reschedule, or cancel your visit are
given. Simple guidelines in accessing and obtaining your medical records are
presented. We have also acquired all the necessary information for you to

The EYE REPUBLIC Ophthalmology Clinic out patient consultation services
operate on a MODIFIED FIRST-COME, FIRST-SERVED basis. There is always an eye doctor in
each clinic so you are guaranteed to see one during your visit. Our
modified first-come, first-served policy is enforced in each clinic, so our front desk
staff no longer take appointments by the hour or fraction thereof. We do,
however, provide specific schedules of doctors for your planning
convenience. We also understand that there are instances when a patient
directly contacts our physicians and arranges for special consultations on a
case-to-case basis. These patient-physician arrangements are honored by each
clinic as best as possible.

Preregistering expedites your clinic consultation. A time range for your
actual arrival on a particular day is given. Preregistration is still
subject to our modified first-come, first-served policy.
Note: Due to
the popularity of our online preregistration feature, the online
preregistration for a clinic visit must be completed 1-2 weeks ahead of
your scheduled appearance. For over-the-phone preregistration for a clinic
visit, it must be completed 2-3 days ahead of your scheduled

patients in to the schedule quickly if they have active disease or
sight-threatening complications. If you intend to walk into any of our
branches, we suggest that you do so as early as your schedule permits in

of Ophthalmology and Visual Sciences Emergency Department to respond to
clinical emergencies involving patients with eye disease. If your
situation is an emergency, rather than trying to use the hospital's
appointment scheduling system, we recommend that you go to the Emergency
Department for emergency care and referral to our clinic and doctors. The PGH
Department of Ophthalmology and Visual Sciences Emergency Department is open

EYE REPUBLIC Ophthalmology Clinic - Asian
Hospital works closely with the Asian
Hospital and Medical Center Emergency Department to respond to clinical
emergencies involving patients with eye disease. If your situation is an
emergency, rather than trying to use the hospital's appointment scheduling
system, we recommend that you go to the Emergency Department for emergency
care and referral to our clinic and doctors. The Asian Hospital and Medical Center

EYE REPUBLIC Ophthalmology Clinic -
Medical City works closely with the Medical City Emergency Department and
resident ophthalmology staff to respond to clinical
emergencies involving patients with eye disease. If your situation is an

system, we recommend that you go to the Emergency Department for emergency
care or ask to be connected to the ophthalmology resident's call room for a referral to our clinic
and doctors. The

St. Luke's Medical Center works closely with the St. Luke's Medical Center Emergency Department and
resident ophthalmology staff to respond to clinical
emergencies involving patients with eye disease. If your situation is an
emergency, rather than trying to use the hospital's appointment scheduling
system, we recommend that you go to the Emergency Department for emergency
care or ask to be connected to the ophthalmology resident's call room for a referral to our clinic
and doctors. The
St. Luke's Medical Center Emergency Department is open 24 hours a day. The phone number is (632)

If you need to change or cancel your
clinic visit, please let us know as
soon as possible. It makes a real difference to our resources if we know you
will not be attending a clinic or operation, and someone else can use your
time.
The more notice you can give us the better, but
we will need 72 hours to register and change your schedule.
Our
dedication to excellent medical record keeping
All our eye
consultants spend a lot of time paying attention to detail and listing them
down carefully in a standardized paper chart. We back this up digitally
daily, and attach it to our digital charts. Laboratory results are scanned
and digital photos are appended to records. We return all hard-copy
laboratory results to our patients and do not keep them on file. We provide
our patients with a records folder and advise them to bring them along
during each visit. We strongly believe in
maintaining this dedication to excellent medical record keeping. By doing
so, we are able to hold true to our vision statements.
How to
Obtain Copies of Your Medical Record
We need a
written request from you or your physician for a copy of your medical
records.
There may be
a charge involved if your request is for legal or insurance reasons. (The
current fee as of 01/06/06 is PhP224.00 plus PhP2.50 per page of printed
materials.) If your request is for further medical treatment, there is no
charge.
It can take
4-6 weeks to receive the requested information if you were a patient here
more than a few years ago since the information has to be obtained from
archived records.

Our Eye MDs
are Philippine Health
Insurance Corporation (PHIC) Accredited Health Care Providers. Asian
Hospital, Medical City, and St. Luke's accepts PhilHealth plan holders.
If your visit or procedure is covered by insurance the EYE REPUBLIC
Ophthalmology Clinic will bill your insurer directly. Please bring your
insurance card and forms with you to your appointment. EYE REPUBLIC
Ophthalmology Clinic's Registration Department can also verify clinic visit
and insurance information in advance by calling up the specific clinic you
wish to visit.
Please make
sure that any referrals or pre-authorizations required by your insurance
have been obtained in advance of the service. You will be billed for all
applicable co-payments and deductibles allowed by your plan. If we are unable
to verify your insurance at the time of service you will be asked to sign an
insurance waiver and assume responsibility for your bill until the necessary
data is obtained by the EYE REPUBLIC Ophthalmology Clinic. EYE REPUBLIC
Ophthalmology Clinic physicians, partners of
the Ophthalmic Consultants Philippines Co., are individually accredited with PhilHealth and many commercial carriers.
International, cosmetic and self pay patients are asked to provide a deposit
based on an estimate of charges. The balance of payment is due upon
discharge. If your procedure is not covered by insurance, an EYE REPUBLIC
Ophthalmology Clinic
financial counselor will be happy to work out a payment plan with you.
Patients who are seen at the EYE REPUBLIC Ophthalmology
Clinic
will receive a bill from the clinic
The clinic charges a facility fee which covers the costs
of running the facility. The current facility fee is PhP224.00. Some health
plans cover the facility fee. The clinic bill may also include charges for
diagnostic and laboratory tests, radiology services, optical products,
prescription drugs, and supplies used in patient care.
If you are seen in the Emergency Room you may be asked to pay applicable
co-payments for Emergency Services at the time of the visit. The physician
bill for professional services will be charged by the EYE REPUBLIC
Ophthalmology Clinic. If your health
plan has an office co-payment you may be asked to pay that amount at the
time of your appointment.
If you have questions about your health benefits, please
call your insurer to ask if a service is covered and to determine if there
are any co-payments, coinsurance, or deductibles for which you will be
responsible.
Both the clinic and physician group accept cash only. For
hospital payments, credit cards may be accepted. If you need assistance

Please have your billing statement(s) available. This
will allow staff to more effectively assist you.
Your hospital
bill will include charges for all hospital services including operating and
recovery room costs, nursing and technical personnel and supplies. It may
also include charges for services requested by your physician such as
laboratory tests, radiology, drugs, supplies, therapies, and consultations.
Your surgeon,
anesthesiologist, and any other medical consultant will be billed to you
separately by the physician's billing office.
